On March 15, 2015, at an unknown time, a Piper PA-34, Ecuadorian registration HC-CNC, impacted terrain under unknown circumstances shortly after takeoff from Edmundo Carvajal Airport, Macas, Ecuador. The pilot, the sole occupant, was fatally injured. The cargo flight was destined for Taisha, Ecuador. The investigation is under the jurisdiction of the government of Ecuador.
